On page 1 the following documentation must be provided:

Tax period : in this section you must indicate the start and end date of the period to which the declaration corresponds.

Declaration identification : the labels previously provided by the AEAT (NIF, company name and identification codes) must be pasted here.

Exercise : In this box you must indicate what the financial year is like, putting 1, 2 or 3 depending on the situation:

Financial year of 12 months that coincides with the calendar year.
Financial year of 12 months that does not coincide with the calendar year.
Financial year lasting less than 12 months.
Next, you must fill out the annex to the form:

CNAE code: in this box you must enter the code corresponding to netherlands mobile phone number data the activity carried out by the company.

Supplementary declaration : this box should only be checked if the declaration is being made to replace a previous incorrect one.
